First, the game must decide that a sacrifice room will spawn that floor. Depending on the dice roll on the floor there is different effects. If you're having an amazing run and find a dice room that you don't know the effet of, i'd be careful, it may shuffle you to worse items. On a dice room with 1 pip it shows the d4 because in the game the d4 active item rerolls all your items. On a 2 pip room, it shows the d20 because the d20 active item rerolls all pickups (cards, pulls, coins etc).
